Description:

RESPONSIBILITIES 1. Assessment and treatment planning for assigned patients. a. Development of Initial Treatment Plan and review of Psychosocial history in first 72 hours. b. Coordination of assessments and completion of Integrated Clinical Summary and Master Treatment Plan (MTP) within the first ten (10) days. c. Review of patient progress weekly and Completing and presenting a 30-day MTP/Phase I review to client and team members at point of transition from Phase I to Phase II. 2. Counseling of assigned patients. a. Individual, group and family counseling as assigned. b. Document individual, group and family counseling sessions in TIER. c. Assignment of life story, first step inventory, second and third step worksheets and presentation of assignments by primary clients as scheduled through intensive outpatient phase of treatment. 3. Case Management of assigned patients a. Manage a caseload of assigned patients. b. Responsible for completion of documentation according to MARR Policy and Procedure, JCAHO and ORS requirements,, including daily and weekly progress notes for all primary clients assigned during Phase I. c. Participating as member of the interdisciplinary treatment team to ensure communication and patient care. d. Provide advocacy for patients and liaison with employers, courts, family or others as required. 4. On-call duties as assigned. 5. Participates in supervision, staff meetings, patient staffings, and other meetings as assigned. 6. Coordinates coverage with other counselors to make sure that groups are staffed. 7. Qualifications:

QUALIFICATIONS: Master’s degree preferred in a counseling related field, Certified Addiction Counselor (e.g. CAC) and/or licensing as a counselor in the State of Georgia (e.g. LAPC, LPC, LMSW, LMFT), or a Counselor in Training (CIT) or actively pursuing licensure. Experience working in the field of addiction treatment. Strong organizational skills. Good communication skills. Ability to work as a member of a team.
